South East London Community Blood Pressure Screening

Are you a community group/organisation, GP, Primary Care Network (PCN) or pharmacy team interested in collaborating to improve blood pressure detection and treatment for south east London residents and explore innovative approaches?
A resource has been developed by south east London community groups and general practice clinicians to support blood pressure testing in community settings. This resource will support teams of community members and clinicians to:
- Provide health education and support the early detection of high blood pressure;
- Deliver effective and culturally sensitive blood pressure checks;
… with the aim to reduce the impact of cardiovascular disease e.g. strokes and heart attacks, within underserved communities most affected by health inequalities.
The South East London Integrated Care Board (SEL ICB) are inviting local community groups in south east London to partner with general practice, PCN and pharmacy teams in their area to roll out this tool and capture the learning. Applicants must already have agreed to work in collaboration i.e. a community group must have a clinical partner. The funding will be paid to community organisations.
There are grants available (circa £3.5k) for community groups to engage in this work.
